• HMSA HMO, PPO, Federal, Blue Card

• HMSA Quest (unfortunately we are not currently accepting new HMSA Quest patients)

• UHA

We also accept self-pay patients should you wish to pay out of pocket. We can provide detailed billing transcripts for submission to your insurance company or FSA.
